Reminder: New versions of the Insurance Application and Policy Change Form

We would like to remind you that the Insurance Application (FIND0035A) and the Policy Change Form (FIND0116A) have been updated. The new versions (2016-12) are available to order since December 12, 2016.

We invite you to use our "Printed Documents Order Form" accessible via the illustration software or the advisor extranet site to obtain the new versions.

Once you receive the new versions, we ask that you dispose of any previous versions that you may still have.

Previous versions of applications and policy change forms without evidence of insurability will be accepted up until February 28, 2017.
