Reminder: New Application Forms and Amendment to the Information Folder and Contract

NEW APPLICATION FORMS

The new application forms (FRA641) must be used as of January 1, 2014. We have modified Section 3 of the Application Form FRA641 by adding a new question to comply with the new requirement to obtain the projected purpose and nature of the relationship for new enrolments.

Please note that the TFSA application form (FRA1251) has also been updated with only minor changes and you may continue using the current version of the form or order the new version, as per your convenience.

In order to comply with the obligations regarding the information that must be collected on corporations and other entities, the Verification of the Existence (Identity) of Corporations and Other Entities (FRA1235A) form was changed. These changes will enable us to obtain the documents and/or information required to confirm the accuracy of the information obtained regarding the ultimate ownership and control of the entity. This form is available in PDF format only and will be added to our secure Web site in the next few weeks.

 

NEW AMENDMENT TO THE INFORMATION FOLDER AND CONTRACT

Given the opening of two new funds, a new Amendment to the Information Folder and Contract (MRA674 2014-01) must be provided to all new investors as of January 13, 2014, along with the Information Folder (BRA1620 2013-06) that is currently in effect.
